Mechanism of Action	Rocuronium bromide is a nondepolarizing neuromuscular blocking agent that competes with acetylcholine for binding to nicotinic acetylcholine receptors at the motor end-plate. This action is antagonized by acetylcholinesterase inhibitors such as neostigmine.
Pharmacokinetics	– Absorption: Poorly absorbed when taken orally. Following intravenous administration, absorption is essentially complete.<br>- Distribution: Distributes into the extracellular space. Central volume of distribution is 78 ± 24 mL/kg.<br>- Metabolism: Primarily hepatically metabolized to a less active metabolite, 17-desacetyl-rocuronium.<br>- Elimination: Half-life is 1.4 to 2.4 hours. Excreted primarily in bile and urine.
Indications	– Adjunct to general anesthesia for rapid sequence and routine tracheal intubation.<br>- Provides skeletal muscle relaxation for surgery.<br>- Facilitates mechanical ventilation in critically ill patients.
Administration	Administered intravenously. Dosing varies based on clinical indication and patient characteristics. For example, 0.6 mg/kg is used for endotracheal intubation, and 1 to 1.2 mg/kg for rapid sequence intubation.
Special Populations	– Hepatic Impairment: Prolonged duration of action due to delayed elimination.<br>- Renal Impairment: No dose adjustment needed.<br>- Pregnancy: Can be used in the 2nd and 3rd trimesters with limited human data indicating no fetal harm.<br>- Pediatric Patients: Dosing is similar to adults but adjusted based on age and anesthesia type.
Monitoring	Monitor depth of neuromuscular blockade using a peripheral nerve stimulator to assess twitch response.
Reversal Agents	– Anticholinesterases: Neostigmine can reverse the effects of rocuronium when twitch response is present.<br>- Sugammadex: Encapsulates rocuronium to reverse neuromuscular blockade rapidly and effectively.
Contraindications	– Allergic reaction to rocuronium.<br>- Use in unsedated patients to avoid awareness.<br>- Use as an infusion to prevent critical illness myopathy and polyneuropathy.
