Indications	– Local anesthesia for minor surgical procedures.<br>- Used for local infiltration and peripheral nerve block.<br>- Not intended for spinal or epidural anesthesia.
Pharmacokinetics	– Rapidly absorbed from the injection site.<br>- Metabolized in the liver to para-aminobenzoic acid (PABA) and other metabolites.<br>- Excreted primarily in urine.
Dosage Forms	Available as sterile solutions in concentrations of 1% and 2% for injection.
Administration	– For local infiltration: 0.25% to 0.5% solutions, up to 350-600 mg total dose.<br>- For peripheral nerve blocks: 0.5%, 1%, and 2% solutions, up to 200 mL, 100 mL, and 50 mL, respectively.<br>- For neuraxial anesthesia: 10% solution, doses ranging from 50 to 200 mg.
Adverse Effects	– Hypersensitivity reactions (e.g., urticaria, pruritus, erythema, laryngeal edema).<br>- Systemic toxicity (LAST) can cause cardiac (e.g., arrhythmias, heart failure) and neurological (e.g., seizures, coma) effects.<br>- Other side effects include nausea, vomiting, dizziness, and syncope.
Contraindications	– Hypersensitivity to procaine or its components.<br>- Patients with cardiac, renal, or hepatic impairment.<br>- Myasthenia gravis.<br>- Pseudocholinesterase deficiency.<br>- Systemic lupus erythematosus (SLE).<br>- Pregnancy and breastfeeding.
Special Considerations	– Use with caution in elderly or acutely ill patients.<br>- Monitor for signs of systemic toxicity, especially in patients with reduced metabolism.<br>- Avoid use in patients with a history of PABA allergy.
