| Mechanism of Action | Procainamide is a Class 1A antiarrhythmic agent that binds to fast sodium channels, inhibiting recovery after repolarization. It prolongs the action potential, reduces impulse conduction speed, and decreases myocardial excitability and contractility. |
| Clinical Uses | – Treatment of ventricular arrhythmias, supraventricular arrhythmias, atrial flutter, atrial fibrillation, AV nodal re-entrant tachycardia, and Wolff-Parkinson-White syndrome.<br>- Used for chemical cardioversion in atrial flutter and atrial fibrillation.<br>- Effective in terminating ventricular tachycardia in patients with structural heart disease. |
| Administration | – IV Loading Dose: 10-17 mg/kg at a rate of 20-50 mg/min.<br>- Maintenance Dose: 1-4 mg/min.<br>- Oral Dosing: 50 mg/kg/24 hours divided into doses every 6 hours. |
