Mechanism of Action – Prazosin is a selective α1-adrenergic receptor antagonist that causes vasodilation by blocking the action of noradrenaline on vascular smooth muscle, leading to decreased blood pressure.<br>- It also crosses the blood-brain barrier and may modulate sleep patterns, making it useful for PTSD-related nightmares.
Indications – Treatment of hypertension (high blood pressure).<br>- Management of benign prostatic hyperplasia (BPH).<br>- Treatment of PTSD, particularly for nightmares and sleep disturbances.<br>- Used in Raynaud’s syndrome.
Pharmacokinetics – Absorption: Oral bioavailability of 50-70%, peak plasma concentration reached in 1-3 hours.<br>- Distribution: Volume of distribution 0.5-1.5 L/kg, highly protein-bound (97%).<br>- Metabolism: Metabolized in the liver via demethylation and conjugation.<br>- Excretion: Half-life of 2-3 hours, primarily excreted in bile and feces.
Dosage Forms – Oral capsules available in 1 mg, 2 mg, and 5 mg.
Administration – Hypertension: Initial dose 1 mg three times daily, maintenance dose 1-20 mg/day.<br>- BPH: Initial dose 1 mg three times daily, maintenance dose 2-10 mg/day.<br>- PTSD: Initial dose 1 mg at bedtime, titrated up to 3-15 mg/day.<br>- Raynaud’s Syndrome: Initial dose 0.5 mg twice daily, maintenance dose 1-2 mg twice daily.
Adverse Effects – Common: Dizziness, headache, drowsiness, nausea.<br>- Serious: First-dose hypotension, orthostatic hypotension, syncope, priapism.<br>- Other: Palpitations, weakness, depression.
