| Mechanism of Action | Pantoprazole sodium is a proton pump inhibitor (PPI) that irreversibly binds to the (H+, K+)-ATPase enzyme system on the secretory surface of gastric parietal cells. This binding inhibits both basal and stimulated gastric acid secretion, regardless of the stimulus. |
| Clinical Uses | It is used for the short-term treatment of various gastrointestinal conditions, including duodenal ulcers, gastric ulcers, gastroesophageal reflux disease (GERD), and pathological hypersecretory conditions such as Zollinger-Ellison syndrome. It is also used to prevent gastrointestinal lesions induced by nonsteroidal anti-inflammatory drugs (NSAIDs) and to manage ulcers associated with Helicobacter pylori infection. |
| Storage Conditions | – Pantoprazole sodium 4 mg/mL stored in glass vials is stable for 3 days at room temperature (20°C to 25°C) with exposure to light.
- When stored in polypropylene syringes under refrigeration (2°C to 8°C) with protection from light, it is stable for 28 days.
- Diluted to 0.4 mg/mL in 5% dextrose in water (D5W) and stored in PVC minibags, it is stable for 2 days at room temperature or 14 days under refrigeration.
- At 0.8 mg/mL in D5W, it is stable for 3 days at room temperature or 28 days under refrigeration.
- Diluted to 0.4 or 0.8 mg/mL in 0.9% sodium chloride (NS) and stored in PVC minibags, it is stable for 3 days at room temperature or 28 days under refrigeration. |
| Special Precautions | – Do not use concurrently with medicines containing rilpivirine (e.g., Complera®, Edurant®, Odefsey®) to avoid adverse effects.
- Monitor for signs of acute tubulointerstitial nephritis, such as changes in urination frequency, blood in urine, fever, joint pain, nausea, or unusual fatigue.
- Long-term use may lead to vitamin B12 deficiency, increased risk of fractures, hypomagnesemia, and fundic gland polyps.
- Serious skin reactions and systemic lupus erythematosus may occur; monitor for joint pain, skin rash, or other symptoms.
- Do not stop taking pantoprazole without consulting a doctor. |
