Mechanism of Action Oxaliplatin is an alkylating agent that binds to DNA, forming cross-links that inhibit DNA replication and transcription, leading to cell cycle arrest and apoptosis. It works synergistically with fluoropyrimidines like 5-fluorouracil.
Pharmacokinetics Oxaliplatin undergoes rapid non-enzymatic biotransformation and is not metabolized by the CYP450 enzyme system. It has a triphasic elimination with a significant terminal half-life. Approximately 50% is excreted in urine and 2% in feces.
Administration Administered via intravenous infusion over 2 hours. In cases of acute toxicity, the infusion may be extended to 6 hours. Pre-treatment with antiemetics is recommended to prevent chemotherapy-induced nausea and vomiting.
Adverse Effects Common side effects include peripheral sensory neuropathy, nausea, vomiting, diarrhea, abdominal pain, constipation, anemia, thrombocytopenia, leukopenia, and elevated liver enzymes.
Contraindications Contraindicated in patients with hypersensitivity to platinum compounds, grade 3 or 4 peripheral sensory neuropathy, severe renal impairment, and during pregnancy or breastfeeding.
Monitoring Regular blood tests, urine tests, liver and renal function tests, and electrocardiograms are required to monitor patients. Neurological exams are also essential to assess for oxaliplatin-induced peripheral neuropathy.
Drug Interactions Oxaliplatin may interact with various drugs, including baricitinib, chloramphenicol, clozapine, QT-prolonging agents, and taxane derivatives, potentially enhancing adverse effects or diminishing therapeutic efficacy.
Storage Conditions Oxaliplatin should be stored at 4°C, protected from light. In solvent, it can be stored at -80°C for 6 months or -20°C for 1 month.
Off-label Uses Used off-label for advanced biliary adenocarcinoma, esophageal and gastric cancers, refractory neuroendocrine tumors, non-Hodgkin lymphoma, advanced ovarian cancer, pancreatic cancer, and testicular cancer.
