| Mechanism of Action | Nepafenac is a prodrug that is rapidly converted to its active metabolite, amfenac, in ocular tissues. Amfenac inhibits cyclooxygenase (COX-1 and COX-2) enzymes, reducing the synthesis of prostaglandins and thereby decreasing inflammation and pain. |
| Indications | – Used to treat ocular pain and inflammation associated with cataract surgery.
- May also be used for other ophthalmic conditions as determined by a healthcare provider. |
| Dosage Forms | 0.1% ophthalmic suspension (eye drops) |
| Administration | – Administered topically to the eye.
- Shake well before use.
- If using multiple eye medications, administer at least 5 minutes apart. |
| Pharmacokinetics | – Rapidly crosses the cornea, achieving higher intraocular concentrations compared to conventional NSAIDs.
- Metabolized to amfenac primarily in the iris, ciliary body, retina, and choroid. |
| Side Effects | – Common: Secondary cataract, decreased visual acuity, foreign body sensation, increased intraocular pressure, sticky feeling in the eye.
- Serious: Allergic reactions (e.g., swelling, difficulty breathing), delayed healing, corneal complications. |
| Precautions | – Not recommended for patients allergic to NSAIDs or any components of the product.
- Avoid contamination of the eye drop container to prevent infections.
- Do not wear contact lenses during treatment.
- Monitor for delayed healing, especially when used with corticosteroids. |
