| Special Considerations | – Titration: Administer slowly to achieve the desired effect. Complete reversal of high opioid doses can result in total reversal of analgesia and other adverse effects, including hypertension and agitation.<br>- Cardiovascular Risks: Use with caution in patients with preexisting cardiovascular disease. Naloxone can cause hypotension, hypertension, ventricular tachycardia/fibrillation, and pulmonary edema.<br>- Opioid Withdrawal: May precipitate severe opioid withdrawal symptoms, especially in patients physically dependent on opioids. Symptoms include nausea, vomiting, sweating, tremor, tachycardia, hypertension, and seizures.<br>- Recurrent Depression: Monitor patients closely for recurrent respiratory and CNS depression, as the duration of action of most opioids may exceed that of naloxone.<br>- Accidental Needlestick: Caution with prefilled syringes to avoid accidental needlestick injuries.<br>- Pregnancy and Lactation: Use only when clearly needed in pregnant women, as naloxone crosses the placenta and may cause fetal distress. Caution is also advised in nursing mothers. |
