Mechanism of Action	– Mirtazapine is a tetracyclic antidepressant (TeCA) that acts by inhibiting central presynaptic α₂-adrenergic receptors, leading to increased release of serotonin and norepinephrine.<br>- It also acts as a potent antagonist of histamine-1 (H₁) receptors, 5-HT₂A, 5-HT₂C, and 5-HT₃ serotonin receptors, contributing to its sedative, antiemetic, anxiolytic, and appetite-stimulating effects.<br>- Unlike SSRIs, mirtazapine does not inhibit the reuptake of serotonin, dopamine, or norepinephrine.
Clinical Uses	– FDA-Approved Indications: Major depressive disorder (MDD).<br>- Off-Label Uses: Insomnia, panic disorder, posttraumatic stress disorder (PTSD), obsessive-compulsive disorder (OCD), generalized anxiety disorder (GAD), social anxiety disorder, chronic tension-type headache, fibromyalgia.
Pharmacokinetics	– Absorption: Rapidly absorbed, with peak plasma concentrations reached within 2 hours.<br>- Distribution: High plasma protein binding (85%).<br>- Metabolism: Metabolized by cytochrome P450 enzymes (CYP1A2, CYP2D6, CYP3A4) in the liver.<br>- Elimination: Mean elimination half-life of 20-40 hours; metabolites excreted primarily via urine (75%) and feces (15%).
Adverse Effects	– Common side effects include drowsiness, dry mouth, increased appetite, weight gain, and sedation.<br>- Rare but serious side effects include agranulocytosis and neutropenia.<br>- No significant cardiovascular effects at high doses.
Toxicity	– Relatively safe in overdose, with few fatalities reported.<br>- Symptoms of overdose include disorientation, drowsiness, bradyarrhythmias, and impaired memory.
