Mechanism of Action Memantine is a low-to-moderate-affinity, voltage- and use-dependent NMDA receptor antagonist. It modulates the activity of glutamate, a neurotransmitter involved in learning and memory, by binding to the NMDA receptor and preventing excessive calcium influx, which is associated with neuronal damage in Alzheimer’s disease.
Clinical Uses It is used for the treatment of moderate to severe Alzheimer’s disease to improve cognitive function and delay disease progression.
Formulations Available as immediate-release tablets/capsules (5 mg, 10 mg) and extended-release capsules (7 mg, 28 mg). The extended-release formulation allows for once-daily dosing.
Pharmacokinetics – Absorption: Well-absorbed orally, with peak plasma concentrations reached in 3–7 hours.<br>- Distribution: Mean volume of distribution is 9–11 L/kg.<br>- Metabolism: Primarily excreted unchanged in urine (48–82%). Metabolized to three main metabolites with minimal NMDA receptor activity.<br>- Elimination: Terminal half-life is approximately 60–80 hours.
Special Precautions – Use with caution in patients with renal impairment. Dose adjustment is required for severe renal impairment (creatinine clearance 5–29 mL/min).<br>- Monitor patients for potential adverse effects such as dizziness, headache, and constipation.<br>- Avoid concurrent use with other central nervous system depressants.<br>- Not recommended for patients with a history of epilepsy or seizure disorders.<br>- Pregnant and breastfeeding women should use with caution.
Side Effects Common side effects include dizziness, headache, constipation, and hypertension. Serious side effects may include hallucinations, confusion, anxiety, and renal dysfunction.
