Mechanism of Action Itopride HCl is a prokinetic agent that enhances gastrointestinal motility through two primary mechanisms: <br>- Dopamine D2 Receptor Antagonism: Blocks dopamine D2 receptors, preventing dopamine from inhibiting acetylcholine release, thereby promoting gastric motility.<br>- Acetylcholinesterase Inhibition: Inhibits the enzyme acetylcholinesterase, increasing acetylcholine levels in the gastrointestinal tract, which stimulates smooth muscle contractions and enhances peristalsis.
Indications – Functional dyspepsia.<br>- Gastroesophageal reflux disease (GERD).<br>- Non-ulcer dyspepsia.<br>- Delayed gastric emptying.<br>- Nausea and vomiting.
Dosage The usual adult dose is 150 mg per day, divided into three doses of 50 mg each, taken before meals.
Contraindications – Hypersensitivity to itopride or any of its components.<br>- History of gastrointestinal perforation, bleeding, or obstruction.<br>- History of prolactin-secreting pituitary tumors.
Special Precautions – Use with caution in patients with hepatic or renal impairment.<br>- Monitor for adverse reactions in elderly patients.<br>- Caution in pregnant or breastfeeding women, as safety data are limited.<br>- Avoid long-term use without improvement of symptoms.
