Chemical Structure Dolasetron mesylate monohydrate is a selective serotonin subtype 3 (5-HT3) receptor antagonist. Its empirical formula is C19H20N2O3·CH3SO3H·H2O, with a molecular weight of 438.50.
Chemical Properties – White to off-white powder.<br>- Freely soluble in water and propylene glycol, slightly soluble in ethanol and normal saline.
Mechanism of Action Acts as a selective 5-HT3 receptor antagonist, reducing nausea and vomiting by blocking serotonin released from enterochromaffin cells in the small intestine.
Indications – Prevention and treatment of postoperative nausea and vomiting (PONV) in adults and children aged 2 years and older.<br>- Not recommended for routine prophylaxis in patients with low risk of PONV.
Administration – Intravenous: Can be infused rapidly (within 30 seconds) or diluted and infused over up to 15 minutes.<br>- Oral: For pediatric patients, the injection solution can be mixed with apple or apple-grape juice.
Dosage – Intravenous: 12.5 mg dose is effective for preventing PONV.<br>- Pediatric Oral: 1.2 mg/kg up to a maximum of 100 mg, given within 2 hours before surgery.
Storage Conditions Store at 20-25°C (68-77°F) with excursions permitted to 15-30°C (59-86°F). Protect from light.
Contraindications – Hypersensitivity to dolasetron.<br>- Intravenous use is contraindicated for preventing nausea and vomiting associated with cancer chemotherapy due to QT prolongation.
Special Considerations – ECG monitoring recommended for elderly and renally impaired patients.<br>- Stability: Diluted solution is stable at room temperature for 24 hours or under refrigeration for 48 hours.
Adverse Effects – Common: Headache, dizziness, somnolence.<br>- Rare: QT prolongation, arrhythmias
