Mechanism of Action Carbamazepine works by modulating voltage-gated sodium channels, reducing the generation of action potentials and synaptic transmission. It also binds to other voltage-gated ion channels, such as calcium channels.
Indications – Treatment of epilepsy, specifically partial seizures, generalized tonic seizures, and mixed seizure patterns.<br>- Management of trigeminal neuralgia.<br>- Acute manic and mixed episodes in bipolar I disorder.<br>- Off-label uses include treatment of neuropathic pain, fibromyalgia, refractory schizophrenia, and restless leg syndrome.
Dosage Forms – Tablets (100 mg, 200 mg, 300 mg, 400 mg)<br>- Chewable tablets (100 mg, 200 mg)<br>- Oral suspension (100 mg/5 mL)<br>- Extended-release capsules (100 mg, 200 mg, 300 mg)<br>- Extended-release tablets (100 mg, 200 mg, 400 mg).
Administration – Initial dosing for adults: 200 mg twice daily, gradually increased to a minimum effective level.<br>- For children under 12: 100 mg twice daily.<br>- Extended-release tablets should not be crushed or chewed.
Adverse Effects – Common: Dizziness, drowsiness, ataxia, nausea, vomiting.<br>- Serious: Severe dermatologic reactions (e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis), agranulocytosis, aplastic anemia, hepatotoxicity, cardiac dysfunction.<br>- Other: Hyponatremia, increased intraocular pressure, urinary retention.
Contraindications – Bone marrow depression.<br>- Hypersensitivity to carbamazepine or tricyclic compounds.<br>- Concurrent use with MAOIs or within 14 days of stopping MAOIs.<br>- Use with nefazodone.
Special Considerations – Monitor for severe skin reactions, especially in patients of Han Chinese ancestry.<br>- Regular blood tests and liver function monitoring.<br>- Use with caution in pregnancy and breastfeeding due to potential teratogenic effects.<br>- Avoid abrupt discontinuation to prevent seizure recurrence.
