| Introduction | Capreomycin Sulphate is a polypeptide antibiotic used primarily in the treatment of multidrug-resistant tuberculosis (MDR-TB). It is a second-line drug that inhibits protein synthesis in Mycobacterium tuberculosis by binding to the 70S ribosomal unit. |
| Mechanism of Action | Capreomycin Sulphate binds to the 70S ribosomal unit of Mycobacterium tuberculosis, inhibiting protein synthesis and thus preventing bacterial growth. |
| Medical Uses | – Multidrug-Resistant Tuberculosis (MDR-TB): Used in combination with other drugs.
- Research: Studied for its anti-amyloidogenic and pro-fibrinolytic activities, with potential applications in Alzheimer’s disease and Parkinson’s disease. |
| Dosage Forms | – Injection: Administered intramuscularly or intravenously.
- Powder for Inhalation: Investigated for use in inhalation therapy. |
| Common Side Effects | – Nephrotoxicity: Elevated blood urea nitrogen (BUN), decreased creatinine clearance, proteinuria.
- Ototoxicity: Hearing loss, tinnitus, vestibular dysfunction.
- Other: Headache, fatigue, respiratory infections. |
| Serious Side Effects | – Severe Nephrotoxicity: Significant kidney damage requiring dose adjustment or discontinuation.
- Permanent Hearing Loss: Risk of irreversible ototoxicity.
- Neuromuscular Blockade: Potential for respiratory paralysis. |
| Precautions | – Renal Function Monitoring: Regular renal function tests are required.
- Hearing Tests: Regular hearing assessments to detect ototoxicity.
- Avoid Concomitant Use: Do not use with other nephrotoxic or ototoxic drugs.
- Special Populations: Caution in elderly patients and those with renal impairment.
- Pregnancy and Breastfeeding: Use with caution; potential risk to the fetus. |
| Storage Conditions | Store at -20°C for powder; in solvent, store at -80°C. |
| Contraindications | – Hypersensitivity to Capreomycin.
- Severe renal impairment.
- Pregnancy and breastfeeding unless benefits outweigh risks. |
