Capreomycin sulfate is used to study the interaction of bacterial ribosomal subunits and the transport process during protein synthesis. It is effective for a variety of gram-positive and Gram-negative bacteria, especially for Bifidobacterium. Anti tuberculosis drugs.

Introduction Capreomycin Sulfate is a polypeptide antibiotic isolated from Streptomyces capreolus. It is used primarily to treat tuberculosis (TB), especially in cases where other first-line drugs are ineffective or the bacteria are resistant.
Mechanism of Action Capreomycin is thought to inhibit bacterial protein synthesis by binding to the 70S ribosomal unit. It also binds to bacterial cell components, leading to the production of abnormal proteins that are fatal to the bacteria.
Medical Uses It is indicated for pulmonary infections caused by capreomycin-susceptible strains of Mycobacterium tuberculosis when primary agents (e.g., isoniazid, rifampin) are ineffective or cannot be used due to toxicity or resistance.
Dosage Forms Available as an injectable solution for intramuscular or intravenous administration. It must be reconstituted before use.
Dosage The usual dose is 1 g daily (not to exceed 20 mg/kg/day) for 60 to 120 days, followed by 1 g 2 or 3 times weekly. Dosing adjustments are required for patients with renal impairment.
Storage Conditions Reconstituted solutions may be stored for up to 24 hours under refrigeration.
Common Side Effects Ototoxicity (hearing loss), nephrotoxicity (kidney damage), and electrolyte imbalances (e.g., hypokalemia, hypomagnesemia).
Serious Side Effects Severe renal impairment, auditory damage, and peripheral neuropathy. Caution is needed in patients with preexisting auditory impairment or renal insufficiency.
Precautions Regular audiometric and renal function tests are recommended. Use with other ototoxic or nephrotoxic drugs should be undertaken with caution.
Contraindications Hypersensitivity to capreomycin. Not recommended for pediatric use or in patients with severe renal impairment.
