Cabazitaxel (CAS No.: 183133-96-2), a semi-synthetic taxane derivative, is a pivotal microtubule inhibitor approved for metastatic castration-resistant prostate cancer (mCRPC) treatment. Marketed as Jevtana®, it binds to β-tubulin subunits, stabilizing microtubules and arresting cells in the G2/M phase. Clinical trials (e.g., TROPIC Phase III) demonstrated superior overall survival (15.1 vs. 12.7 months) compared to mitoxantrone when combined with prednisone. Its mechanism involves PI3K/Akt/mTOR pathway-induced autophagy and CYP3A4 metabolism, necessitating caution with CYP3A inhibitors. Notably, cabazitaxel retains activity against docetaxel-resistant cancer cells, making it a critical second-line option. Structurally, it features a 7β,10β-dimethoxy substitution, enhancing lipophilicity and tumor penetration.
